KVKK Application Form

Pursuant to the first paragraph of Article 13 of the KVK Law, applications regarding these rights submitted to our Company, as the data controller, must be delivered to us in writing or by other methods determined by the Personal Data Protection Board (the “Board”).

In this context, applications made to our Company “in writing” may be submitted by printing out this form and delivering it:

• By the Applicant’s personal application,
• Through a notary public,
• By the Applicant using a “secure electronic signature” as defined in the Electronic Signature Law No. 5070, a mobile signature, or an email address that you have previously notified to our Company and that is registered in our systems.

Application Channels

Below is information on how written applications should be delivered to us, organized by application channel:

• Personal application (with a document verifying your identity): Bonair Havacılık Ticaret ve Sanayi A.Ş., Şenlikköy Mah. Orman Sk. No:10, 34153 Bakırköy / İstanbul — Write “Information Request Under the Personal Data Protection Law” on the envelope.
• Service via notary public: Bonair Havacılık Ticaret ve Sanayi A.Ş., Şenlikköy Mah. Orman Sk. No:10, 34153 Bakırköy / İstanbul — Write “Information Request Under the Personal Data Protection Law” on the notification envelope.
• Via secure electronic signature, mobile signature, or an email address registered in our systems: bonair@hs02.kep.tr or kvkk@bonair.com.tr — Write “Personal Data Protection Law Information Request” in the email subject line.

A. Applicant Contact Information

Please provide the following information in your application:

• Name
• Surname
• T.R. Identity Number
• Phone Number
• E-mail
• Address

B. Your Relationship with Our Company

Please indicate your relationship with our Company (Customer, Employee, Supplier, Business partner, Other), the unit you are in contact with, and the subject of your application. Where applicable, also state:

• If you are a former employee, the years you worked,
• If you submitted a job application / shared your CV, the date,
• If you are a third-party company employee, the company you work for and your position.

C. Your Request

Please describe your request under the KVK Law in detail.

D. Method of Response

Please select the method by which you would like to receive our response to your application:

• I would like it sent to my address.
• I would like to collect it in person. (In the case of collection by proxy, a notarized power of attorney or authorization document is required.)

This application form has been prepared in order to identify your relationship with our Company and, where applicable, to fully determine the personal data processed by our Company about you, so that an accurate and legally timely response can be provided to your application. In order to eliminate legal risks that may arise from unlawful and improper data sharing, and in particular to ensure the security of your personal data, our Company reserves the right to request additional documents and information (such as a copy of your ID card or driver’s license) to verify your identity and authority. In the event that the information provided in this form is inaccurate or not up to date, or that an unauthorized application is made, our Company accepts no liability for requests arising from such incorrect information or unauthorized application.
